FAITHWorks Community Wellness Center

Christian churches have had a meaningful historic link to clinical healthcare dating back to the 19th century.  Located within the new Emmanuel Baptist Church building, this bold collaborative project with the FAITHWorks Wellness Group, LLC, will create a full-service community health resource that will include tierd fee schedules, open-scheduling (walk-in appointments), urgent care services as well as counseling, life-coaching and social work services.  The fee schedule will be tiered based on income and other parameters but will reduce significant practice overhead associated with collections and claims filing.  The health center will be recognized as both a Federally Qualified Health Center (FQHC) as well as a Patient Centered Medical Home (PCMH); serving also as a loan repayment site for physicians, advance practice clinicians (PAs and NPs) and nurses.  Digital radiography, CLIA-waived point-of-care testing will streamline clinical data collection; while technology-based platforms will make care completely customizable including virtual clinical visits and consultation.

            Estimated Cost:  $450,000

Estimated Jobs Created:  5 (professional); 3 (support)
